Celebrate Your Child’s Milestones

As part of the Make the First Five Count campaign, Easter Seals DuPage & Fox Valley is proud to offer a free online Ages and Stages Questionnaire (ASQ) for children birth to age five thanks to the generous support of CVS Health. The ASQ will showcase your child's developmental milestones while uncovering any potential delays. Please note that milestones are continuous throughout a child's first five years of life. It is important to keep up with your child's progress by taking the ASQ every three months.

The ASQ contains 30 questions  and takes about 15 minutes to complete. Once finished, the results are scored and reviewed by a member of the Easter Seals DuPage & Fox Valley team. Within seven days, you will receive a copy of the scores along with a letter containing suggestions.

Screen your children every six months to ensure that they are meeting their milestones!

Take the Questionnaire             Tomar el Cuestionario

Best used with Google Chrome, or Mac Safari web browsers.

By detecting developmental delays early, you have the power to change lives and educational outcomes for children! If delays are identified, Easter Seals DuPage & Fox Valley can offer the support needed to be school-ready and build a foundation for a lifetime of learning. Research proves that when children receive the right treatment and therapy they need before age five, they are more prepared to learn alongside their peers, build lifelong skills and achieve their dreams.

Please note: ASQ-3™ is designed for screening, not diagnosis. It is a quick check for children from birth through age five. If your child is age six or older, please discuss his or her development with your child's health care provider, your local school district, or your child's teacher. Your results from the questionnaire will be emailed to you within two weeks.
